There is a plugin for WordPress called "FormBuilder". Give it a try. Or you can try a commercial solution called FormLogix. Good luck!

I work with MacBooks every day....both with the small one and with the Pro version. If you have the money I'd say definitely buy the Pro. The Pro is better in all aspects and the weight is about the same between the two. Go Pro!
